Free-Format address block

I've seen this somewhere on your site, but I can't find it anymore.
I'm looking for ready to use free-format questions that can be used for survey participants to enter their address, name, phone, etc.
Simple verification of email (contains "@" and ".") would be great.

Go into Sawtooth under HELP/SAMPLE STUDIES/CIW SAMPLE. Look at the free format question at the end called "info". It contains an example where name and address, etc are collected.
