Unable to connect to sql 2008 r2 database

I have uploaded a survey to the server.  Activeperl is installed and the DBD-ODBC and DBI packages have been installed. I created a regular odbc connection to the database using the ip and port and was able to connect successfully.

When I open the admin page I get the following error.

Ive modified the error so real IP's and login info isnt displayed.  The server where the database resides has a named instance where the survey database will be.

Sorry but this is the first survey Ive tried to configure so any tips would be helpful.

We just got the same error on a Sawtooth survey. (several times)

We are also getting this message when we attempt to log into the admin page:

Are you using your own server? Or are you hosting with Sawtooth Software?
We are using our own server.  We are trying to reach the server admin.
You are probably out of memory on the server and need more hardware.
One other thing I have noticed is that we are getting an repeating error message that says,
Error #199 - Failed to connect to the database.
Server Error: DBI connect('deftr2_client_projects:127.0.0.1','deftr2_clients',...) failed: Can't connect to MySQL server on '127.0.0.1' (111) at /home/deftr2/public_html/deftsurvey.com/mxs/cgi-bin/authlib8_4_2.pl line 17

When I look on the server, I see a file named named authlib8_3_6.pl  but not one named authlib8_4_2.pl    Could that be the problem?  

I was running version 8.3.6.   A colleague created and uploaded this survey using v 8.4.2 .  I just updated to v8.4.4.
Our client_projects database is nearly 5 GB in size.  Should we be creating a new project database for every project or once every few months?   

I ran a db repair on that database and it gave me a timeout when it got to one of the tables for the current study.  it then completed the repair and now it checks out as OK.   Would you advise switching to the new database?
