How to I create a response list that forces the respondent to choose one answer per pair, from several pairs?

To clarify-

Q1 (multi-select) offers the respondent 3 pairs of responses:
- Pair 1, with answer choices A and B
- Pair 2, with answer choices C and D
- Pair 3, with answer choices E and F

I want to the respondent to choose one answer from Pair 1, one answer from Pair 2, and one answer from Pair 3.

How do I create a question with several paired response options? Also, how do I create the question such that only one answer can be chosen from each pair?

Alternative 1:

Create three select question on same page with mutually exclusion between two choices. First with choice A & B, second with C & D, third with E & F.

Display options in each question in two cols so that it appears as one question.

In data, you will have 6 variables.

Alternative 2:

Write javascript to restrict responses.
